Terms of Service | Heart of Longevity

These Terms of Service ("Terms") govern your use of mylongevitymirror.com (the "Site") and any program, service, or event offered by Heart of Longevity and The Prevent Clinic (used interchangeably, "we," "us," "our"), led by Gina Pritchard, DNP. By using this Site, submitting a Discovery Application, enrolling in a program, or purchasing a Bootcamp ticket, you agree to these Terms. If you do not agree, do not use the Site or purchase our services.

1. Who We Are

Heart of Longevity is the public movement and The Prevent Clinic is the clinical practice behind it, founded by Gina Pritchard, DNP in 2005. Our office is located at:

Heart of Longevity operates under Now and Forever Faith Inc. Dr. Gina Pritchard is the clinical authority and provides or supervises the clinical care delivered through this practice.

You must be at least 18 years old to use this Site, submit a Discovery Application, or enter into any agreement with us.

2. The Nature of This Relationship, Please Read Carefully

Using this Site, joining our email or text list, submitting a Discovery Application, or booking a call does not, by itself, make you a patient and does not create a doctor-patient relationship. A doctor-patient relationship begins only after a formal clinical intake and evaluation with Dr. Gina or a member of her clinical team acting within her supervision and license. Until that intake happens, nothing you submit through this Site is treated as part of a medical record, and nothing on this Site is a diagnosis, a treatment recommendation, or medical advice specific to you.

Telehealth and licensure limits. Dr. Gina Pritchard provides telehealth care only in the states and jurisdictions where she holds active licensure, currently 47 states and the District of Columbia. She is not licensed in Alaska, Missouri, or South Dakota, and we cannot enroll or provide clinical care to residents of those three states at this time. If you are a resident of one of those states, or move to one after enrolling, we will let you know and will not be able to continue providing care there.

Team-delivered care. Some portions of your care plan (accountability check-ins, community facilitation, client care support) may be delivered by members of Dr. Gina's team rather than by Dr. Gina personally. Any clinical judgment, diagnosis, or prescribing decision is made by Dr. Gina or by a licensed provider acting within her supervision and applicable state scope-of-practice rules.

No medical advice, no guarantee of outcome. Nothing on this Site, in any program material, or in any conversation with our team is a substitute for your own physician's advice or should delay you from seeking emergency care. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room. Individual results vary and depend on your own health history, testing, and adherence to any care plan. We do not guarantee any specific health outcome, including prevention, slowing, or reversal of any disease or condition. Any educational materials we publish are provided for general educational purposes only and are not medical advice.

Referrals to independent third-party providers. From time to time, evaluation of your case may result in a referral to an independent third-party provider outside Dr. Gina's direct care, for example a stem cell or regenerative-medicine provider, or another licensed prescriber where a specific therapy falls outside Dr. Gina's prescriptive authority in your state. Any such referral is a referral only. We do not administer, sell, warrant, or guarantee the outcome of care provided by an independent third party, and that provider's own terms and privacy practices apply to your relationship with them, not these Terms.

3. Acceptable Use of the Site

You agree to use this Site only for its intended purpose: learning about our programs, submitting accurate information through our forms, and, if appropriate, enrolling in a program or event. You agree not to:

Submit false, misleading, or another person's information through the Discovery Application, Opt-In form, or Bootcamp registration without their knowledge and consent
Attempt to gain unauthorized access to any part of the Site, our systems, or any account
Scrape, harvest, or copy content from the Site for any commercial purpose without our written permission
Use the Site to transmit anything unlawful, harassing, or harmful to others
Interfere with the Site's normal operation, including through automated bots, denial-of-service attempts, or similar activity

We may suspend or terminate your access to the Site or any account if you violate this section.

4. Intellectual Property

The Site, including its text, design, graphics, and the frameworks and program names described below, is owned by or licensed to Heart of Longevity / The Prevent Clinic and is protected by copyright, trademark, and other applicable laws.

The following are proprietary names and frameworks of Heart of Longevity / The Prevent Clinic, used with a trademark-style claim of ownership:

You may not copy, reproduce, distribute, or create derivative works from our program materials, curriculum, frameworks, or branded content without our prior written permission. This includes materials you receive as part of an enrolled program. They are licensed to you for your personal use in your own care, not for redistribution or commercial use.

5. Payment Terms

"My Longevity Mirror Breakthrough," annual program, billed quarterly

Billed annually as four equal quarterly payments, at the current pricing disclosed to you before you enroll. This program includes quarterly lab testing (blood and saliva, four times a year), the included equipment provided as part of the program (a Continuous Glucose Monitor, the Heads Up Health app, and an Oura Ring), monthly 1:1 meetings with Dr. Gina, private calendar access, weekly accountability check-ins, a customer service line and dedicated client care specialist, Skool community access and weekly live group sessions, a new 90-day care plan each quarter, and inclusion in the four Heart of Longevity Bootcamps held during the program year. Nothing described above is billed separately.

Monthly Consultation & Calendar Access, alternative plan

Billed monthly, at the current pricing disclosed to you before you enroll, for ongoing consultation access: Dr. Gina's private calendar link, two 45-minute sessions a month (extended to 1.5 hours if there are items to review), access to the custom care line, a dedicated client care specialist, Skool community, and live group sessions. This is a separate offering from the annual program above, not an add-on to it.

Heart of Longevity Bootcamp

A ticketed event with General Admission and VIP tiers, priced as shown to you at the time of ticket purchase. VIP benefits are as described to you at the time of purchase. Clients currently enrolled in the annual "My Longevity Mirror Breakthrough" program receive Bootcamp access at no additional cost and should contact their care team rather than purchase a ticket.

Recurring and automatic billing disclosure. Both the quarterly annual-program plan and the monthly consultation plan involve recurring, automatic charges to the payment method you provide. By enrolling, you expressly authorize us to charge that payment method on the applicable recurring schedule until you cancel in accordance with Section 8 below. We will clearly disclose the total cost, billing frequency, and cancellation method to you before you complete enrollment, and we will not make it harder to cancel than it was to sign up.

Payment processing. Payments are processed through a third-party payment processor, currently Square. We do not store your full payment card information ourselves. Scheduling of calls may be handled through a third-party scheduling service, currently Calendly. Each of these services has its own terms and privacy practices.

6. Program Participation and Your Responsibilities

You agree to provide accurate and complete information in the Discovery Application and any clinical intake, and to update us if your health information, medications, or state of residence changes materially during your enrollment. Providing inaccurate information may affect the quality of guidance we can give you and, in the case of state of residence, may affect whether we are legally able to continue serving you.

Your care plan, recommendations, and any equipment provided are for your personal use only and should be used as directed by Dr. Gina or her team. This program is not a substitute for care from other physicians you see, and you should continue seeing your other treating physicians unless specifically advised otherwise.

7. No Guarantee of Results, Individual Results Vary

We do not promise, warrant, or guarantee that you will achieve any specific health result, including prevention, detection, slowing, or reversal of heart attack, stroke, diabetes, dementia, or any other condition. Testimonials, if and when we publish any, reflect one individual's experience and are not a representation of what you should expect. Any specific numbers, projections, or outcome language used in our marketing materials are illustrative, not a promise, and individual results vary based on your own biology, adherence, and circumstances.

8. Refund and Cancellation Policy

8A. "My Longevity Mirror Breakthrough" (annual program, billed quarterly)

First-quarter satisfaction window. You may cancel within 7 calendar days of your first quarterly payment and receive a full refund of that payment, provided you have not yet received shipped equipment (Oura Ring) or completed your first 1:1 session with Dr. Gina.
After the 7-day window, each quarterly payment already made is non-refundable. By the time a quarter is billed, real clinical and team resources for that quarter, including lab testing, monthly 1:1 meetings, and care-team time, have already been committed.
Future quarters carry no long-term lock-in. You may cancel your enrollment at any time, effective at the end of your current paid quarter, by providing written notice to [email protected] at least 15 days before your next quarterly billing date. No further quarterly payments will be charged after a valid cancellation notice is received in time.
Equipment on cancellation. The Oura Ring is a physical device already shipped to you and is yours to keep once cancellation happens after it's been sent. Continuous Glucose Monitor units and app access are consumed or used during your active term and are not subject to return.
Missed or failed payments. If a scheduled quarterly payment fails and is not resolved within 10 business days after notice, we may pause your access to the program until payment is resolved, or treat the non-payment as a voluntary cancellation effective at the end of your last paid quarter.

8B. Monthly Consultation & Calendar Access

First-month satisfaction window. You may cancel within 3 calendar days of your first monthly payment and receive a full refund, provided you have not yet used a session or accessed the private calendar link.
After that window, or after your first session, the current month's payment is non-refundable.
No long-term commitment. This plan is month-to-month. Cancel at any time by providing written notice at least 5 business days before your next monthly billing date to stop future charges. No cancellation fee.

8C. Heart of Longevity Bootcamp (General Admission / VIP, event: November 9, 2026)

Full refund if canceled at least 14 days before the event, on or before October 26, 2026.
50% refund, or full credit toward a future Heart of Longevity event, your choice, if canceled 7 to 13 days before the event, October 27 through November 1, 2026.
No refund inside 7 days of the event (November 2, 2026 or later). Free ticket transfer is available at any time up to the event. You may give or sell your ticket to someone else at no charge by notifying us of the new attendee's name and contact information.
If we change the event date after you've purchased a ticket, your ticket is automatically honored on the new date at no additional charge. If the new date does not work for you, you are entitled to a full refund on request, with no time-window restriction.
Current annual-program clients receive Bootcamp access as an included program benefit, not a ticket purchase. This refund policy does not apply to them; their access follows the annual program's own cancellation terms in Section 8A.

9. Limitation of Liability

To the fullest extent permitted by law, Heart of Longevity, The Prevent Clinic, Gina Pritchard, DNP, Chris Cartwright, and Now and Forever Faith Inc. are not liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of the Site or participation in any program, including loss of data, loss of income, or personal injury not resulting from our own negligence. Nothing in this section limits liability for gross negligence, willful misconduct, or any liability that cannot be limited under applicable law.

Where liability is not otherwise excluded, our total liability to you for any claim arising from these Terms or your participation in a program is limited to the amount you actually paid us in the twelve months before the event giving rise to the claim.

We are not responsible for the acts, omissions, terms, or privacy practices of independent third parties, including third-party payment processors (Square), scheduling services (Calendly), community platforms (Skool), equipment manufacturers (Oura), or any independent provider we refer you to.

10. Termination

We may suspend or end our relationship with you, including ending your enrollment in any program, if you fail to make a required payment and don't resolve it within a reasonable period after notice, you violate Section 3 (Acceptable Use), you provide materially false information that affects our ability to safely or lawfully serve you, or continuing your care is, in Dr. Gina's clinical judgment, no longer clinically appropriate.

Consistent with standard medical-practice norms against patient abandonment, if we end a clinical relationship for a reason other than your own request or non-payment, we will provide reasonable advance written notice, at least 30 days, and reasonable assistance identifying alternative care, except where immediate termination is necessary for safety reasons.

You may end your relationship with us at any time in accordance with the cancellation terms in Section 8.

11. Dispute Resolution and Governing Law

These Terms are governed by the laws of the State of Texas, without regard to its conflict-of-laws principles, matching our business address in Frisco, Texas.

Informal resolution first. If you have a concern or dispute, please contact us at [email protected] first. Most concerns can be resolved directly without a formal process.

Venue. If a dispute cannot be resolved informally and legal action becomes necessary, you and we agree that the exclusive venue for any such action will be the state or federal courts located in Collin County, Texas, and you consent to personal jurisdiction there.

12. Changes to These Terms

We may update these Terms from time to time as our programs, pricing, or practices change. The effective date at the top of this page will always reflect the most recent update. Continued use of the Site or continued enrollment in a program after a change takes effect means you accept the updated Terms. For material changes affecting price or cancellation rights on an active enrollment, we will provide direct notice by email before the change applies to your existing enrollment, not just an updated page.
