How we handle your information

Heart of Longevity and The Prevent Clinic (used interchangeably in this policy to refer to the same practice, led by Gina Pritchard, DNP) respect your privacy. This page explains what information we collect through mylongevitymirror.com, why we collect it, how we protect it, how long we keep it, and the rights you have over it.

This policy applies to every form and page on this site that collects your information, including the Opt-In quick-capture form, the Discovery Application, and the Heart of Longevity Bootcamp waitlist. Where a specific form collects more sensitive information than another, that difference is called out below rather than treated the same way across the board.

Who We Are

Heart of Longevity is the public-facing movement and The Prevent Clinic is the clinical practice behind it, founded by Gina Pritchard, DNP in 2005. Our office is located at:

Dr. Gina Pritchard provides telehealth care in the states and jurisdictions where she holds active licensure. Visiting this site, joining our email list, or submitting an application does not by itself make you a patient or create a doctor-patient relationship. That relationship begins only after a formal clinical intake and evaluation, described further below.

What Information We Collect

We collect different information depending on which form you fill out. We've broken this out by form on purpose, because the sensitivity of what we ask for is not the same across every page on this site.

1. Opt-In (Quick Capture) Form

If you click "Begin Your Discovery" or a similar call to action, you'll first land on a short form that asks for:

• Your name
• Your phone number
• Your email address
• Your consent to be contacted by phone, text, or email about our programs

This is contact information only. We do not ask for any health information at this step.

2. Discovery Application

If you continue past the Opt-In form, the fuller Discovery Application asks for more detailed information so we can determine whether our program is a good fit for you and, if so, which program step to discuss first. This includes:

Health information:

• Your top health goals and concerns
• Any testing you've already had (bloodwork, saliva/hormone panels, sleep studies, imaging, genetic testing)
• Notes or records from other doctors you've seen
• Your family health history

Financial and household information:

• Your profession
• Approximate household income
• Whether your household has one or two incomes
• Whether you're enrolling individually or with a spouse/partner

This is a pre-enrollment screening step, not a clinical evaluation. No patient or treatment relationship exists at this stage. We use this information only to evaluate fit and prepare for a conversation with you about next steps, not to diagnose, treat, or make any clinical judgment about you. If you enroll, a separate clinical intake process follows, and that later step (not this form) becomes part of your medical record. Because this form isn't part of your medical record, this policy is not a HIPAA Notice of Privacy Practices. If you become a patient, you will receive a separate Notice of Privacy Practices describing your rights over your clinical record at that time.

3. Heart of Longevity Bootcamp Waitlist

If you join the waitlist for our upcoming Bootcamp event, we collect only your name and email address, so we can notify you when dates, curriculum, and registration open.

4. Information Collected Through Scheduling and Payment

If you book a call with us, we use Calendly, a third-party scheduling service, which will collect the information you provide to schedule that call (typically name, email, and the time you select). If you enroll in a paid program, payment is processed through Square, a third-party payment processor, which collects the payment information you provide directly to them. We do not store your full payment card information ourselves. Each of these services has its own privacy practices, which we encourage you to review.

Why We Collect This Information

We use the information described above to:

• Respond to your inquiry and follow up with you about our programs
• Evaluate whether our program is a good fit for you, and if so, recommend a realistic starting point
• Prepare for a conversation with you about your health goals and program options
• Schedule calls and process enrollment if you decide to move forward
• Send you information you've requested (Bootcamp updates, program details)
• Notify our team internally so your submission can be reviewed promptly

We do not use your information to build advertising audiences, and we do not use your information (including any information about visits to a medical facility) to target ads to you.

How We Store and Protect Your Information

Your information is stored electronically in an access-restricted system and is accessible only to Dr. Gina Pritchard and the members of her team directly involved in reviewing prospective-client screenings and inquiries. We apply reasonable administrative, technical, and physical safeguards, including limiting access on a need-to-know basis and encrypting information in transit between your device and our systems (HTTPS).

A copy of your Discovery Application or Opt-In submission is also sent by email to Dr. Gina Pritchard and to Chris Cartwright (our client care specialist) so your submission can be reviewed promptly. These email copies are a separate channel from the access-restricted system described above and are not covered by the retention window described below.

If we ever experience a security incident that compromises your information, we will notify you as required by applicable state law, without unreasonable delay.

How Long We Keep Your Information

If you enroll in one of our programs, your information becomes part of your file and is retained under the terms that apply to your ongoing care relationship going forward.

If you do not enroll, we keep your information for up to 24 months from the date you submit a form, after which it is deleted, unless you ask us to delete it sooner or we're required to keep it longer by law. This applies to Opt-In submissions, Discovery Applications, and Bootcamp waitlist entries alike. You can ask us to delete your information at any time before then, see "Your Privacy Rights" below.

The 24-month window above is our standard default retention period. It is not tied to any specific legal requirement and can be adjusted; we've applied it consistently across our forms so there is one clear answer rather than a different rule for each page.

We Do Not Sell Your Information

We do not sell your information. We do not share it with third parties for marketing purposes. We do not use it to build advertising audiences, and we do not use location data to target ads to you based on this information, your health information, or your visits to any medical facility.

We do share limited information with the third-party service providers described above (Calendly for scheduling, Square for payment processing, and our email delivery provider for sending notifications and communications) solely so they can perform the specific service we've engaged them for.

Health Information and HIPAA

Some of the information we collect, particularly through the Discovery Application, relates to your health. We want to be direct with you about where things stand:

• We have not yet confirmed whether The Prevent Clinic is a "covered entity" under HIPAA (the federal Health Insurance Portability and Accountability Act). That status depends on billing and payment mechanics that are still being finalized. We are treating your health information with the same care we would if HIPAA applied, whether or not it turns out to apply as a matter of law.
• Regardless of HIPAA status, several states (including Washington, Nevada, and Connecticut) have their own consumer health data laws that apply to information like what we collect on the Discovery Application, whether or not a treatment relationship exists yet and whether or not a practice is a HIPAA-covered entity. Because it isn't yet confirmed which states our clients live in, we apply the rights described below to every person who submits health information to us, regardless of what state you're in.
• This policy, and the consent language on our Discovery Application, is not a HIPAA Notice of Privacy Practices. If you enroll and become a patient, you will receive a separate Notice of Privacy Practices at that time, specific to your clinical record.

Your Privacy Rights

If you have submitted health information to us through the Discovery Application, you have the following rights, regardless of what state you live in:

• Right to know exactly what health information we've collected about you
• Right to access a copy of the health information you've provided us
• Right to delete your health information, other than what we're legally required to retain
• Right to withdraw your consent at any time going forward (withdrawal doesn't undo use of information already reviewed to prepare for a conversation already held)
• Right to non-discrimination, declining to consent, or later withdrawing consent, will not be held against you or change the quality of care or service offered to you

If you have submitted only contact information (through the Opt-In form or Bootcamp waitlist), you have the right to know what we've collected, access it, and ask us to delete it, on the same terms described above.

Text Message and Email Communication Consent

If you check the box on our Opt-In form or elsewhere on this site agreeing to be contacted, you are agreeing to receive calls, texts, or emails from Heart of Longevity / The Prevent Clinic about our programs. Message and data rates may apply, and message frequency varies. Agreeing to be contacted is not a condition of enrolling in any program or receiving care.

You can opt out of text messages at any time by replying STOP, or reply HELP for help.

Cookies and Analytics

We do not currently use cookies, tracking pixels, or web analytics tools on this site. If that changes in the future, we will update this policy to describe what's used and why before it's put into place.

Children's Privacy

This site and our programs are intended for adults. We do not knowingly collect information from anyone under 18. If you believe a minor has submitted information to us, contact us at [email protected] and we will delete it.

Changes to This Policy

We may update this policy from time to time as our programs, tools, or practices change. The effective date at the top of this page will always reflect the most recent update. We encourage you to review this page periodically.
